· Please call and cancel your appointment if you can not make your appointment, ahead of time “24 hours in advance” so we can allocate your time to another patient. · If your health insurance has been changed please inform us and bring your insurance card with you at the time of visit. · Please inform our office if your address or phone number has been changed. · To refill your prescription, it would be easier that you have your pharmacy fax us your medication refill request to my office. However for some medication patient has to be seen and re-examed due to medication side effect before refilling the medication is issued. · Please do not wait until you ran out of medication then ask for refill… Make your request a week prior to ran out of medication. · For none emergency after hour call “if you are my patient and you have been seen before in my office” I need to have the D.O.B. of the patient/your phone number/pharmacy phone number. · During the day and at the time I am with a patient I do not receive or accept any phone calls, I usually respond to all my phone messages late in the afternoon or at the time I have no patient. Patients who have come to my office have higher priority. · My off hour Cell phone number is Restricted/Blocked for reason of privacy. If the phone number that you leave on my voice mail does not accept Restricted/Blocked call, I am sorry I can not return your call. · My office visit for those whom do not have health Insurance is $60.00 cash. @
